Follow these instructions to create a web form:
- Login to your admin page.
- Click "Edit Website".
- Click "Manage Library".
- Click "Web Forms".
- Title your Web Form.
- Enter a password if you desire your Web Form to have it's own password otherwise leave this field blank.
- Click "Add Form"
Now you need to add custom questions to your form:
- Click the title of the form you just created.
- Click "Add Question".
- Check the "Enabled" check box to enable your question. (Note: questions which are not enabled will not appear on the form...)
- Check the "Required" check box if you wish for the question to be a required question. Members cannot submit the form unless all required questions are answered.
- Enter a number between 0 and 255 to sequence your question. Questions with a value of 0 will be displayed first then those with value 1 will be displayed second and so on.
- Enter your question into the "Question" text box.
- Choose your answer type.
- Short Text Box - Creates a Small Text Field
- Long Text Box - Creates a Large Text Field
- Radio Button - Allows you to create multiple choices where the member can only choose one.
- Drop Down - Allows you to create a drop-down menu where the
member can select one or multiple answers by holding the control key.
- If you chose the Radio Button or Drop Down menu then you must enter in answer values into the "Answer Value" field and Click "Add To Answer Value". Add as many answers as you wish and click "Save" and repeat.
- Click "Save" near the bottom of the page.
