- Login to admin.
- Click "Programs".
- Select a program or league by clicking its name.
- Click "Registration".
- Click "Forms".
- Click "Make your own question..." to add player custom questions. Click the "Parent/Guardian" or "Emergency" tab to add custom questions to these categories.
- Title your question by editing the "Question" field.
- Choose your "Answer Type".

• Non-Question - This is if you would like to enter a statement or note between questions
  
• Text-Small - Creates a Small Text Field
• Text-Medium - Creates a Medium Text Field
• Text-Large - Creates a Large Text Field
• Yes/No - Creates a yes or no answer drop down menu
• True/False - Creates a True or False answer drop down menu
• Check Box - Allows you to create multiple check boxes for your members to select
  
• From List - Allows you to create a drop-down menu where the member can select one or multiple answers by holding the control key

- If you would like to require the question choose the "Required" radio button. If you would like it to be option choose the "Optional" radio button.
- Click "Advanced Options" for the Bold and Indent options.
- Click "Save".
